Schedule/Hours:
- Weekend rotation required once every 6 weeks.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 1 year of certified home care experience.
- Minimum of 1 year of case management experience.
- Strong assessment, organizational, and communication skills.
- Proficiency with electronic documentation systems.
- Ability and willingness to travel to assigned patient cases.
Key Responsibilities:
- Assess patient health conditions and needs in their home environment.
- Develop and implement individualized care plans according to organizational philosophies, policies, and community health standards.
- Provide patient and family education, support, and resources as needed to promote optimal health outcomes.
- Perform admission processes as required, ensuring thorough and timely documentation using electronic health records.
- Manage ongoing cases in accordance with established standards of practice, collaborating with interdisciplinary team members to coordinate comprehensive care.
- Facilitate effective communication with patients, families, and healthcare team members.
- Maintain up-to-date records and participate in regular reviews to ensure quality of care.
