Schedule/Hours:

• Weekend rotation required once every 6 weeks.

Qualifications:

• Active Registered Nurse (RN) license.
• At least 1 year of certified home care experience.
• Minimum of 1 year of case management experience.
• Strong assessment, organizational, and communication skills.
• Proficiency with electronic documentation systems.
• Ability and willingness to travel to assigned patient cases.

Key Responsibilities:

• Assess patient health conditions and needs in their home environment.
• Develop and implement individualized care plans according to organizational philosophies, policies, and community health standards.
• Provide patient and family education, support, and resources as needed to promote optimal health outcomes.
• Perform admission processes as required, ensuring thorough and timely documentation using electronic health records.
• Manage ongoing cases in accordance with established standards of practice, collaborating with interdisciplinary team members to coordinate comprehensive care.
• Facilitate effective communication with patients, families, and healthcare team members.
• Maintain up-to-date records and participate in regular reviews to ensure quality of care.
