We are seeking a dedicated Registered Nurse (RN) with proven OASIS experience to provide essential in-home services for a 13-week full-time contract assignment. Schedule & Assignment Details:
- 13-week contract with potential for extension
- Full-time, Monday through Friday schedule (on-call likely required)
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver skilled nursing interventions and comprehensive in-home care
- Accurately assess patients and complete timely OASIS documentation
- Collaborate with interdisciplinary teams and physicians to develop and implement individualized care plans
- Perform wound care, medication administration, education, and chronic condition monitoring
- Communicate important updates and patient status changes promptly to the care team
Desired Qualifications:
- Active, unrestricted RN license
- At least one year of recent home health RN experience is required
- OASIS documentation proficiency is mandatory
- Experience with HomeCare HomeBase is strongly preferred
- Strong organization skills to manage structured daily schedules
- Reliable transportation and a valid drivers license
