A highly skilled Registered Nurse with OASIS experience is needed for a 13-week full-time home health assignment. This role offers the unique chance to deliver critical care in patients homes while building meaningful relationships and advancing your clinical expertise.
Schedule:
- 13-week contract commitment, with possibility of extension
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily structured visit expectations
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and patient education
- Complete timely, accurate patient assessments and OASIS documentation
- Collaborate with physicians and multidisciplinary teams to implement tailored care plans
- Perform wound care, medication management, and continuous disease monitoring
- Maintain clear communication with patients, families, and clinical colleagues regarding condition changes and updates
Qualifications:
- Active Registered Nurse (RN) license
- At least one year of recent RN experience in home health settings
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Ability to manage a structured, full-time caseload and travel between home environments
