Schedule/Hours:

• Flexible schedule tailored around patient needs.

Responsibilities include:

• Delivering comprehensive nursing care within patients homes.
• Conducting thorough initial and ongoing patient assessments, covering vital signs, medication regimens, and wound care.
• Developing, implementing, and updating individualized care plans based on clinical findings and patient needs.
• Providing education for patients and families regarding disease processes, treatments, and preventive practices.
• Collaborating with physicians, therapists, and the wider healthcare team to ensure holistic care.
• Accurately documenting all care activities in electronic medical records (EMR).
• Monitoring patient progress and promptly reporting changes in condition to supervising clinicians.
• Maintaining compliance with all state and federal home health guidelines.

Qualifications:

• Current, active RN license.
• Minimum of 3 months home health experience utilizing OASIS documentation.
• HCHB (HomeCare HomeBase) software experience is highly desirable.
• Strong skills in IV therapy, medication administration, and wound care.
• Comfortable and proficient with EMR systems.
• Excellent communication and interpersonal abilities.
• Capable of working independently and efficiently managing a caseload.
• Valid drivers license with access to reliable transportation.
