We are seeking a dedicated Registered Nurse (RN) with strong OASIS expertise for a 13-week full-time contract. Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily structured patient visit expectations
Key Responsibilities:
- Deliver hands-on, skilled nursing care in patient homes
- Conduct comprehensive assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and healthcare teams to develop and implement patient-specific care plans
- Provide wound care, medication administration, disease monitoring, and patient/family education
- Communicate any changes in patient condition to the clinical care team
Qualifications:
- Current, active RN license
- At least one year of recent home health RN experience (required)
- Proficient with OASIS documentation (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
