Your next nursing opportunity starts here. A dedicated Registered Nurse (RN) with strong home health and OASIS expertise is needed for a 13-week, full-time contract. Schedule & Hours:
- Full-time, Monday through Friday
- On-call may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care, focusing on safety and comfort
- Complete thorough patient assessments and document using OASIS standards
- Create and implement personalized care plans in collaboration with physicians and multidisciplinary care teams
- Provide wound care, manage medications, educate patients and families, and monitor chronic conditions
- Communicate promptly about patient condition changes with the clinical team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proven OASIS documentation skills
- Proficiency with HomeCare HomeBase (strongly preferred)
- Strong organizational skills to manage a busy patient visit schedule
- Reliable transportation and valid drivers license
