A rewarding full-time assignment is available for a skilled Registered Nurse with proven home health and OASIS experience. This 13-week contract offers the chance to support patients in their homes and be a vital part of a dedicated, collaborative care team.
Schedule:
- 13-week assignment, with possibility of extension
- Monday through Friday, full-time hours
- On-call responsibilities may apply
What Youll Do:
- Deliver compassionate, responsive in-home nursing care and skilled interventions
- Accurately assess patients and complete OASIS documentation within required timelines
- Collaborate with coordinating physicians, therapists, and care professionals to achieve optimal patient outcomes
- Perform wound care, medication administration and management, patient and family education, and disease monitoring
- Keep the interdisciplinary team informed of any changes in patient condition and progress
What Were Looking For:
- Active RN license in good standing
- At least 1 year of recent home health RN experience
- Strong OASIS documentation skills are mandatory
- Familiarity with HomeCare HomeBase EMR (highly preferred)
- Ability to manage a structured daily visit schedule
- Reliable transportation and a valid drivers license
