Your next nursing opportunity starts here. We're seeking a dedicated Registered Nurse (RN) with strong home health experience and OASIS proficiency for a 13-week full-time contract assignment. If you thrive in independent settings, excel at documentation, and are passionate about high-quality patient care, this role is an excellent fit.
Schedule:
- 13-week contract, with possibility for extension
- Full-time hours, Monday through Friday
- On-call duties may be required
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions for patients in a home health setting
- Complete comprehensive patient assessments and accurate OASIS documentation on a timely basis
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Provide wound care, patient and family education, medication management, and monitor patient conditions
- Communicate effectively with the clinical team regarding any changes in patient status
Qualifications:
- Active RN license (unrestricted)
- At least 1 year of recent experience as a home health RN
- OASIS documentation experience is required
- Proficiency with HomeCare HomeBase software is strongly preferred
- Dependable transportation and valid drivers license
