Your next professional adventure in nursing awaits! Were seeking a skilled and compassionate Registered Nurse (RN) with strong OASIS experience for a 13-week full-time home health contract. This role is perfect for RNs with proven clinical expertise, exceptional documentation skills, and the ability to efficiently manage daily visits while delivering patient-centered care.
Schedule:
- 13-week contract with potential extension
- Full-time, Monday through Friday
- On-call availability may be required
Core Responsibilities:
- Deliver in-home nursing care, providing skilled interventions tailored to each patients needs
- Complete patient assessments and maintain timely, accurate OASIS documentation
- Work collaboratively with physicians and care teams to create and execute individualized care plans
- Perform wound care, medication management, patient education, and monitor disease processes
- Communicate any patient status changes to the broader clinical team
Qualifications:
- Active RN license and current BLS certification
- At least 1 year of recent RN experience in home health
- OASIS expertise required
- HomeCare HomeBase proficiency strongly preferred
- Ability to manage a structured, full-time caseload with daily visits
- Reliable transportation and a valid drivers license
