A Registered Nurse (RN) with home health experience is needed for a contract position in Prairieville, LA. The ideal candidate will have a passion for patient care and a solid background in home health nursing, with specific experience using OASIS for patient assessments.
Responsibilities:
- Provide comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals for coordinated care.
- Document all patient care activities accurately and promptly in electronic medical records (EMR).
- Monitor and report changes in patient condition to the supervising physician.
- Ensure compliance with all state and federal home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months home health experience utilizing OASIS.
- Experience with Home Care Home Base (HCHB) electronic records is a strong advantage.
- Strong clinical skills in IV therapy, wound care, and medication management.
- Proficient with electronic medical records (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
