Job Description

Registered Nurses (RN) with home health experience are needed for a contract position in Prairieville, LA. This role is ideal for professionals dedicated to providing high-quality patient care in a home setting and who have specific expertise with OASIS documentation.

Key Responsibilities:

  • Deliver comprehensive nursing care to patients in their homes, including initial and ongoing assessments.
  • Monitor vital signs, manage medications, and provide wound care as required.
  • Develop and implement individualized care plans tailored to each patients needs.
  • Educate patients and families on disease management, treatment plans, and preventive measures.
  • Collaborate with physicians, therapists, and other healthcare professionals to maintain coordinated care.
  • Accurately document patient care activities in electronic medical records (EMR).
  • Monitor and report any changes in patient conditions to the supervising physician.
  • Ensure compliance with all state and federal regulations regarding home health care.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months home health experience with proficiency in OASIS.
  • Experience with Homecare Homebase (HCHB) is a strong advantage.
  • Solid clinical skills including intravenous (IV) therapy, wound care, and medication management.
  • Proficiency in electronic medical records (EMR) systems.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time efficiently.
  • Valid drivers license and reliable transportation.
