Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse (RN) with strong home health experience and OASIS assessment skills for a 13-week full-time contract. If you're adept at clinical care, precise documentation, and managing a structured patient route, this role offers the chance to make a meaningful impact in patients homes.

Schedule:

• 13-week engagement, with potential for extension
• Full-time, Monday through Friday schedule
• On-call rotation may be required

Key Responsibilities:

• Deliver comprehensive in-home nursing care, skilled assessments, and interventions
• Complete thorough patient evaluations and OASIS documentation promptly and accurately
• Collaborate with medical providers and the interdisciplinary team to develop and execute individual care plans
• Perform wound care, medication administration, patient and family education, and ongoing disease monitoring
• Provide timely updates regarding patient status to clinical leadership
• Ensure clear, compassionate communication with patients and their families

Required Qualifications:

• Active RN license in good standing
• Minimum of one year recent experience as a home health RN
• Proficiency and recent experience with OASIS documentation (required)
• Familiarity with HomeCare HomeBase (highly preferred)
• Ability to manage a full-time, structured case load
• Reliable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
