Your next nursing opportunity starts here with a full-time, 13-week contract providing essential care in a home health setting. As a Registered Nurse with a passion for home health and a commitment to clinical excellence, youll play a pivotal role in improving patient outcomes in the community.
Schedule:
- Full-time position, Monday through Friday
- On-call duties may be required
What to Expect:
- 13-week contract with possibility for extension
- Structured daily visit expectations
- Work within a professional, collaborative care team
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions to diverse patients
- Complete accurate and timely OASIS documentation during all patient assessments
- Collaborate with physicians and other care professionals to ensure customized patient plans
- Provide wound care, medication management, education, and disease monitoring
- Maintain clear communication on patient updates and changes in condition with the clinical team
Who You Are:
- Active, unencumbered RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated OASIS documentation experience (required)
- Proficiency with HomeCare HomeBase (strongly preferred)
- Comfortable managing a structured schedule with multiple daily visits
- Reliable transportation and valid drivers license
