Step into a rewarding field-based role focused on delivering compassionate, high-quality care in patients homes. This full-time, 13-week contract offers the chance to make a real difference through skilled nursing services within a supportive and collaborative environment.

Schedule & Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Expectation: 30 points per week (typically 35 patient visits per day)

Desired Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent field-based Home Health experience
• Confident using Home Care Home Base (HCHB) electronic documentation system
• Proficient with OASIS assessment and care planning tools
• Demonstrated ability to travel daily within the assigned territory (reliable transportation, valid drivers license, and auto insurance required)

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans tailored to each patient
• Deliver skilled nursing services such as wound care, medication management, IV therapy, and chronic disease support in patients homes
• Monitor patient progress, promptly identify changes in health status, and coordinate with the broader care team and physicians
• Educate patients and families on diagnoses, treatment protocols, medications, and preventive health practices
• Ensure highly accurate, timely documentation within Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and home health aides for thorough and coordinated care
• Engage in case conferences, staff meetings, and ongoing education to maintain best practices and compliance standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
