Key Qualifications and Experience:

• Current, unrestricted RN license
• Proven experience in home health nursing preferred
• Strong clinical assessment, critical thinking, and documentation abilities
• Proficient with home health EMR systems
• Knowledge of Medicare/Medicaid guidelines and OASIS documentation
• Valid drivers license and reliable transportation
• BLS certification required

Schedule:

• Full-time, weekday shifts
• Schedule may include some weekends or holidays as needed

Key Responsibilities:

• Conduct comprehensive patient assessments and create individualized care plans
• Deliver skilled nursing care and patient education in home settings
• Administer medication, perform wound care, and monitor vital signs
• Coordinate patient services with interdisciplinary team members, including therapists and social workers
• Maintain accurate, timely clinical documentation
• Communicate regularly with physicians, family members, and case managers regarding patient progress
• Ensure practices comply with infection control, safety policies, and state/federal regulations
