Schedule & Assignment

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• 30 points per week caseload (typically 35 patient visits per day)
• Daily travel within a defined territory (mileage reimbursement provided)
• Field-based setting in patient homes

Key Responsibilities

• Perform thorough OASIS assessments and develop individualized care plans
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and the interdisciplinary team
• Educate patients and families regarding disease processes, medication regimens, and preventive care
• Ensure accurate, timely documentation in Home Care Home Base (HCHB)
• Work collaboratively with therapists, social workers, and aides to coordinate comprehensive care
• Participate in case conferences, staff meetings, and required training sessions
• Adhere to agency standards and regulatory requirements for documentation and care delivery

Qualifications

• Active RN license in good standing (state of practice)
• At least 1 year of recent field-based Home Health nursing experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
