We are seeking an experienced and dedicated Registered Nurse with strong OASIS documentation skills to deliver high-quality in-home care during a 13-week contract assignment. This full-time position offers the chance to make a tangible difference for patients in the comfort of their own homes, while collaborating closely with a dedicated care team.
Schedule/Hours:
- Full-time, Monday through Friday
- On-call as needed
Requirements:
- Active Registered Nurse license
- At least one year of recent home health nursing experience
- OASIS documentation proficiency is required
- Experience with HomeCare HomeBase is strongly preferred
- Dependable transportation and a valid drivers license
Key Responsibilities:
- Deliver skilled nursing care and interventions in patient homes
- Complete thorough assessments and OASIS documentation in a timely, accurate manner
- Collaborate with physicians and fellow clinicians to develop and implement personalized care plans
- Perform wound care, medication administration, patient education, and monitor disease progression
- Communicate updates and any changes in patient status to the care team
