Your next nursing opportunity starts here. A dedicated Registered Nurse with strong home health and OASIS experience is needed for a 13-week, full-time contract. This role supports patients in their homes through skilled nursing care and thorough, timely documentation, while working alongside a collaborative, professional healthcare team.
Schedule:
- 13-week contract with the potential for extension
- Monday through Friday, full-time
- On-call responsibilities may be included
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions tailored to home health clients
- Perform comprehensive patient assessments and complete OASIS documentation with accuracy
- Partner with physicians and interdisciplinary teams to develop and update individualized care plans
- Provide wound care, manage medications, offer disease process education, and monitor patient conditions
- Communicate any changes or updates in patient status to the clinical team promptly
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- OASIS documentation expertise required
- Experience with HomeCare HomeBase preferred
- Ability to independently manage a structured, full-time schedule
- Reliable transportation and a valid drivers license
