Embark on a rewarding opportunity providing in-home nursing services and care coordination over a 13-week, full-time assignment. This field-based position offers a structured weekday schedule and the ability to make a direct impact by supporting patients in their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Requirements:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent home health experience (field experience highly valued)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• Recent OASIS assessment experience
• Reliable vehicle, valid drivers license, and current auto insurance

Primary Responsibilities:

• Deliver comprehensive OASIS assessments and individualized care plans in patients homes
• Provide skilled nursing care, including wound care, IV therapy, medication management, and chronic disease monitoring
• Track and assess patient progress, reporting any condition changes to the interdisciplinary team and physicians
• Educate patients and caregivers on ongoing care, medications, treatment strategies, and preventive health
• Document all care accurately and promptly using HCHB
• Coordinate closely with therapists, medical social workers, and aides to optimize patient outcomes
• Attend case conferences, staff meetings, and professional education sessions as required
• Ensure compliance with all regulatory standards and agency protocols

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
