Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

What You Will Bring:

  • Current, active RN license in good standing for the state of practice
  • At least 1 year of recent home health nursing experience (field-based preferred)
  • Solid proficiency with Home Care Home Base (HCHB)
  • Demonstrated experience using OASIS assessment tools
  • Reliable transportation, valid drivers license, and active auto insurance

Your Responsibilities:

  • Complete comprehensive OASIS assessments and craft individualized care plans
  • Deliver skilled nursing interventions, including medication administration, wound care, IV therapy, and chronic disease management
  • Monitor patient status, identify changes in condition, and collaborate promptly with the care team and physicians
  • Educate patients and families on disease states, medication regimens, and preventive healthcare
  • Accurately document all care provided using HCHB in a timely manner
  • Work alongside an interdisciplinary team (PT, OT, social workers, aides) to ensure seamless care coordination
  • Attend case conferences, staff meetings, and ongoing professional development sessions as needed
  • Uphold regulatory and agency standards in care delivery and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
