Step into a rewarding Home Health nursing assignment delivering high-quality care to patients in their homes. This full-time, 13-week contract role offers the opportunity to provide hands-on nursing services in a field-based setting, supporting individuals in your local community.

Schedule

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities

• Conduct comprehensive OASIS assessments and develop individualized plans of care
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and management of chronic illness
• Monitor patients for changes in condition, update care plans, and communicate findings with the healthcare team and physicians
• Educate patients and their families about disease processes, treatment regimens, medications, and ongoing preventive care
• Document all care and assessments accurately using Home Care Home Base (HCHB)
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides to ensure a coordinated approach
• Participate in case conferences, staff meetings, and continuing education to maintain best practices
• Adhere to all regulatory and agency standards for documentation and patient care

Qualifications

• Active RN license in Pennsylvania (in good standing)
• Minimum 1 year of recent Home Health nursing experience (field experience strongly preferred)
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments
• Valid drivers license, reliable transportation, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
