Job Description

This field-based opportunity involves providing vital in-home care to patients as part of a dedicated care team. As an RN specializing in Home Health, you will make a meaningful impact by delivering compassionate, evidence-based nursing interventions while traveling within your assigned territory.

Schedule/Hours:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM

Key Duties:

  • Conduct thorough OASIS assessments and develop tailored care plans for each patient
  • Deliver skilled nursing services such as wound care, IV therapy, medication management, and chronic condition support
  • Perform daily visits (35 per day) with an expectation of 30 points per week
  • Closely monitor patient progress, noting changes and collaborating with physicians and other care professionals
  • Educate patients and their families on treatment plans, medications, and preventative measures
  • Document all care provided using Home Care Home Base (HCHB) system
  • Collaborate with an interdisciplinary team, including therapists, case managers, and aides
  • Participate in case conferences, team meetings, and professional development activities
  • Ensure adherence to clinical standards, agency guidelines, and regulatory requirements

Qualifications:

  • Current, active RN license in good standing in the state of practice
  • Minimum 1 year of recent field Home Health experience
  • Expertise with Home Care Home Base (HCHB) and OASIS assessments required
  • Reliable transportation, valid drivers license, and up-to-date auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
