A dedicated Registered Nurse (RN) with strong OASIS experience is needed for a full-time home health assignment. This 13-week contract offers the chance to work hands-on in patient homes, making a direct, meaningful difference for your community while utilizing advanced clinical and organizational skills.
Schedule:
- Full-time commitment, Monday through Friday
- On-call availability probable
- Structured daily patient visits
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and comprehensive assessments
- Accurately complete OASIS documentation in a timely manner
- Develop and implement individualized care plans in coordination with physicians and interdisciplinary team members
- Provide wound care, manage medications, monitor diseases, and ensure effective patient/family education
- Communicate changes in patient status promptly to the clinical team
Required Qualifications:
- Active, unrestricted RN license
- At least one year of recent experience in home health nursing
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase is highly preferred
- Strong time management skills with the ability to meet daily visit requirements
- Access to reliable transportation and a valid drivers license
