Your next nursing opportunity starts herea rewarding full-time assignment as a Registered Nurse specializing in Home Health. This 13-week contract is designed for an RN with expertise in OASIS and a passion for helping patients thrive in their own homes.

Schedule:

• Full-time, Monday through Friday
• On-call rotation likely
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled nursing care directly to patients in their homes
• Complete comprehensive assessments and OASIS documentation accurately and promptly
• Coordinate with physicians and multidisciplinary teams to tailor individualized care plans
• Manage wound care, medication administration, patient education, and monitor disease progression
• Communicate any patient condition changes and ensure timely updates to the clinical team

Qualifications & Skills:

• Active Registered Nurse (RN) license
• At least 1 year of recent RN experience in home health
• Proficient with OASIS documentation (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and valid drivers license
• Confidence in managing a structured schedule and daily patient visits
