Seeking an experienced RN with a background in home health to provide skilled care to patients in a field-based setting. This full-time, 13-week contract offers the opportunity to support individuals within their homes, ensuring quality care and patient-centered outcomes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments to establish and revise individualized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient progress, proactively communicating any changes to the care team and physicians.
• Educate patients and their families on treatment plans, medications, and preventive strategies to facilitate optimal health.
• Maintain thorough, accurate, and timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to coordinate patient care.
• Actively participate in case conferences, staff meetings, and required continuing education.
• Uphold regulatory and agency standards for patient care delivery and documentation.
• Manage a caseload expectation of 30 points per week, typically 35 patient visits per day, with daily travel required (mileage reimbursement provided).

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum of 1 year of recent home health experience (field experience preferred)
• Proficient in Home Care Home Base (HCHB) electronic documentation
• Demonstrated OASIS assessment experience
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
