Take the next step in your nursing career with an engaging field-based opportunity serving home health patients. This 13-week, full-time RN assignment offers the chance to make a meaningful impact through quality patient care and partnership with an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Caseload & Setting:

• Manage approximately 35 patient visits per day (30 points/week) within patients homes.
• Daily travel required throughout your assigned territory (mileage reimbursement provided).

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Evaluate patient progress, recognize and communicate changes in condition to the care team and physicians promptly.
• Educate patients and families on disease processes, treatments, and preventive health practices.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care.
• Participate in case conferences, staff meetings, and ongoing education to enhance clinical skills.
• Ensure all documentation and care delivery meets agency and regulatory standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• At least 1 year of recent experience in Home Health, preferably with direct field exposure.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, current drivers license, and auto insurance required.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
