Job Description

A Registered Nurse (RN) with skilled home health experience is needed for a full-time 13-week contract in Perrysburg, OH. This position offers an opportunity to deliver compassionate nursing care in patients homes while utilizing your expertise in clinical assessments and OASIS documentation.

Qualifications and Desired Experience:

  • Active RN license required
  • Minimum one year of recent home health nursing experience
  • Proficient in OASIS documentation (required)
  • Familiarity with HomeCare HomeBase software strongly preferred
  • Ability to manage a structured weekday schedule with daily patient visits
  • Valid drivers license and reliable transportation essential

  • Communicate patient progress and changes in condition clearly with the clinical team
