We are searching for an experienced Registered Nurse with strong OASIS skills to provide comprehensive home health care on a full-time, 13-week contract assignment. If you bring expertise in home health and thrive in a structured, independent environment, this role offers the opportunity to make a genuine difference.
Schedule:
- Full-time, Monday through Friday
- On-call duties probable
- Daily structured patient visit expectations
What you'll be responsible for:
- Deliver skilled in-home nursing care, including wound care, medication management, and patient education
- Conduct patient assessments and complete timely, accurate OASIS documentation
- Develop and implement individualized care plans through collaboration with physicians and interdisciplinary team members
- Monitor changes in patient condition and communicate updates clearly to the clinical team
- Maintain high standards of clinical documentation and follow strict schedule management
What were looking for:
- Current and active RN licensure
- At least 1 year of recent experience as a home health RN
- Proficient in OASIS documentation (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Able to manage a consistent, full-time patient schedule
