An opportunity is available for an experienced Registered Nurse (RN) skilled in Home Health, working on a contract basis in the Paterson, NJ area. This role is designed for someone who is passionate about caring for patients in their homes and possesses solid clinical abilities, particularly in OASIS assessment and documentation.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in a home setting.
- Conduct initial and ongoing patient assessments, including vital signs monitoring, medication administration, and wound care.
- Develop and implement individualized care plans tailored to each patients unique needs.
- Provide education for patients and families on treatment plans, disease management, and preventive care strategies.
- Work collaboratively with physicians, therapists, and other healthcare professionals to coordinate comprehensive care.
- Accurately document all care activities and updates using electronic medical records (EMR).
- Regularly monitor patient conditions and promptly report significant changes to the supervising physician.
- Maintain compliance with state and federal home health care regulations.
Qualifications:
- Current, active RN license.
- At least 3 months of home health experience with demonstrated OASIS proficiency.
- Experience with HomeCare HomeBase (HCHB) is highly desirable.
- Strong clinical competencies including IV therapy, wound care, and medication management.
- Proficient in utilizing EMR systems.
- Exceptional communication and interpersonal skills.
- Self-motivated with excellent time management skills.
- Valid drivers license and reliable transportation.
