An opportunity is available for a skilled Registered Nurse (RN) specializing in home health to provide high-quality patient care in a home setting on a contract basis. This role focuses on delivering personalized nursing services and requires expertise in OASIS assessments to ensure accurate patient evaluations and care planning.
Responsibilities:
- Deliver comprehensive nursing care to patients within their homes, including initial and ongoing assessments.
- Monitor vital signs, manage medications, and perform wound care with precision.
- Develop and implement individualized care plans tailored to each patients unique needs.
- Educate patients and family members about disease management, treatment options, and preventive health measures.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to optimize patient outcomes.
- Maintain thorough and accurate patient documentation using electronic medical records (EMR).
- Observe and report any changes in patient condition promptly to supervising physicians.
- Ensure adherence to all applicable state and federal regulations governing home health services.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum three months experience in home health nursing with proficiency in OASIS assessments.
- Experience with Homecare Homebase (HCHB) software is highly desirable.
- Strong clinical skills in IV therapy, wound care, and medication administration.
- Proficient with electronic medical records (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time efficiently.
- Valid drivers license and reliable transportation.
