A skilled Registered Nurse (RN) with home health expertise is sought for a contract role providing patient-centered care in home settings. This position requires hands-on experience in OASIS assessments and delivering comprehensive nursing services to promote patient well-being and independence.
Key Responsibilities:
- Conduct initial and ongoing patient evaluations, including vital signs monitoring, medication administration, and wound care management.
- Develop and execute personalized care plans tailored to individual patient needs.
- Educate patients and family members on disease processes, treatment regimens, and preventive strategies.
- Collaborate closely with physicians, therapists, and interdisciplinary teams to ensure continuity of care.
- Maintain timely and accurate documentation of all nursing interventions in the electronic medical records (EMR).
- Monitor patient conditions vigilantly and report significant changes to supervising physicians.
- Adhere strictly to state and federal guidelines governing home health nursing practice.
Qualifications and Desired Experience:
- Active Registered Nurse (RN) license.
- Minimum of three months experience in home health nursing utilizing OASIS assessments.
- Familiarity with HCHB electronic documentation system is highly advantageous.
- Strong clinical competencies, including intravenous therapy, wound care, and medication management.
- Proficient in EMR usage.
- Excellent communication and interpersonal skills.
- Ability to work autonomously with strong time management.
- Valid drivers license and dependable transportation.
