A dedicated Registered Nurse with expertise in Home Health is needed for a 13-week, full-time contract. This role is ideal for clinicians with strong clinical assessment skills, OASIS documentation experience, and the ability to effectively manage a daily schedule. Youll make a significant difference in patients lives while working autonomously and as part of a collaborative professional team.
Schedule:
- 13-week assignment with potential for extension
- Full-time hours: Monday through Friday (on-call requirements probable)
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled nursing care in patient homes, ensuring safe and high-quality interventions
- Complete patient assessments and OASIS documentation promptly and accurately
- Develop and implement individualized care plans with the healthcare team
- Provide wound care, medication administration, patient education, and chronic disease monitoring
- Effectively communicate patient status updates to the multidisciplinary team
Required Qualifications:
- Current, active RN license
- Minimum 1 year of recent experience as a Home Health RN
- Proficiency with OASIS documentation is required
- Familiarity with HomeCare HomeBase software is strongly preferred
- Reliable transportation and valid drivers license
- Ability to maintain a structured full-time schedule with consistent patient visits
