A dedicated Registered Nurse with expertise in Home Health is needed for a 13-week, full-time contract. This role is ideal for clinicians with strong clinical assessment skills, OASIS documentation experience, and the ability to effectively manage a daily schedule. Schedule:

  • 13-week assignment with potential for extension
  • Full-time hours: Monday through Friday (on-call requirements probable)
  • Structured daily visit expectations

Key Responsibilities:

  • Deliver skilled nursing care in patient homes, ensuring safe and high-quality interventions
  • Complete patient assessments and OASIS documentation promptly and accurately
  • Develop and implement individualized care plans with the healthcare team
  • Provide wound care, medication administration, patient education, and chronic disease monitoring
  • Effectively communicate patient status updates to the multidisciplinary team

Required Qualifications:

  • Current, active RN license
  • Minimum 1 year of recent experience as a Home Health RN
  • Proficiency with OASIS documentation is required
  • Familiarity with HomeCare HomeBase software is strongly preferred
  • Reliable transportation and valid drivers license
  • Ability to maintain a structured full-time schedule with consistent patient visits
