A rewarding position is available for a Registered Nurse with expertise in home health care and OASIS documentation for a full-time, 13-week contract assignment. This opportunity combines a structured schedule with meaningful contributions to patient health in a supportive, professional environment.
Schedule:
- 13-week contract, with potential to extend
- Full-time, Monday through Friday
- On-call coverage may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing and interventions tailored to each patient
- Perform comprehensive patient assessments and complete OASIS documentation accurately and on schedule
- Coordinate care plans with physicians and interdisciplinary care teams
- Provide wound care, medication management, patient education, and chronic disease monitoring
- Communicate significant patient status updates to clinical leadership
Qualifications:
- Current, active RN license
- At least 1 year of recent RN home health experience required
- Proven OASIS documentation experience
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to manage a full-time schedule and consistently meet visit goals
- Reliable transportation and valid drivers license required
