Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse (RN) with proven OASIS experience to provide exceptional in-home care for a full-time, 13-week contract. Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may occur
• Structured daily visit expectations

Key Responsibilities:

• Deliver skilled in-home nursing care in accordance with individualized care plans
• Accurately complete patient assessments and OASIS documentation in a timely manner
• Perform wound care, administer medications, provide patient education, and monitor disease status
• Communicate condition changes and care updates with the interdisciplinary clinical team
• Collaborate closely with physicians and other healthcare professionals to optimize patient outcomes

Qualifications:

• Active RN license required
• Minimum of 1 year recent home health RN experience
• OASIS proficiency is essential
• HomeCare HomeBase experience strongly preferred
• Capable of maintaining a structured, full-time patient visit schedule
• Reliable transportation and a valid drivers license
