A full-time contract position is available for an experienced Registered Nurse (RN) specializing in Home Health care. This 13-week assignment offers the chance to make a real impact in the lives of patients, working independently while supported by a professional care team. Schedule:
- 13-week contract with potential for extension
- Monday through Friday, full-time hours (on-call may be required)
- Daily visit expectations
Key Responsibilities:
- Deliver comprehensive in-home nursing services and skilled interventions
- Conduct thorough patient assessments with timely and accurate OASIS documentation
- Collaborate closely with physicians and multidisciplinary care teams to provide customized care plans
- Perform wound care, medication administration, patient teaching, and monitor for disease progression
- Respond promptly to changes in patient condition, communicating updates to the clinical team
Required Qualifications:
- Active, unrestricted RN license
- At least 1 year of recent home health RN experience
- OASIS documentation proficiency is required
- Skilled in using HomeCare HomeBase (strongly preferred)
- Comfortable managing a busy, set schedule with daily patient interactions
- Reliable transportation and a valid drivers license
