Schedule/Hours:
- Contract position; specific details to be discussed during the interview process.
Key Qualifications:
- Active RN license in good standing
- At least 3 months of home health experience utilizing OASIS (required)
- Familiarity with HCHB is highly desirable
- Solid clinical expertise including IV therapy, wound care, and medication management
- Experience using electronic medical record (EMR) systems
- Outstanding communication and interpersonal skills
- Capable of working independently and managing a flexible case load
- Valid drivers license along with reliable transportation
Primary Responsibilities:
- Deliver holistic nursing care to patients in their homes
- Conduct comprehensive patient assessments, including monitoring vital signs and managing medications and wound care
- Develop, execute, and adjust individualized care plans in collaboration with the patient and care team
- Provide patient and family education regarding treatment, disease management, and preventive strategies
- Communicate and coordinate with physicians, therapists, and other care professionals for effective care delivery
- Maintain accurate, timely documentation in EMR systems
- Diligently monitor and report any patient condition changes
- Ensure compliance with home health regulations at all levels
