An exciting opportunity awaits a skilled Registered Nurse with Home Health experience looking to make a direct impact on patient outcomes in a field-based setting. This full-time, 13-week contract role provides the autonomy to deliver personalized care to patients in their homes while collaborating with an interdisciplinary team.

Schedule and Work Setting

• Monday to Friday, 8:00 AM 5:00 PM
• Daily travel within an assigned territory (mileage reimbursement provided)
• Field-based: 35 patient visits per day, targeting a 30-point caseload per week

Key Qualifications

• Active RN license in good standing (state of practice)
• Minimum 1 year of recent Home Health nursing experience, including field work
• Strong proficiency with Home Care Home Base (HCHB) for documentation
• OASIS assessment and documentation experience required
• Reliable transportation, valid drivers license, and current auto insurance

Core Responsibilities

• Complete thorough OASIS assessments and craft tailored patient care plans
• Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, promptly reporting changes to the care team and physician
• Provide vital education to patients and families regarding disease management, medication, and care procedures
• Maintain up-to-date, accurate documentation in HCHB
• Work closely with physical therapy, occupational therapy, medical social work, and home health aide professionals
• Engage in staff meetings, case conferences, and continuous education opportunities
• Ensure compliance with regulatory and clinical standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
