Take your nursing expertise to the next level with a dynamic opportunity specializing in home health care. This full-time, 13-week contract role offers a rewarding way to deliver skilled care to patients in their own homes, utilizing advanced tools and a collaborative team environment.

Schedule & Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans for patients.
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease support.
• Monitor and evaluate patient progress, quickly identifying changes and communicating with the interdisciplinary team and physicians.
• Educate patients and families regarding disease processes, care plans, and preventive strategies.
• Complete timely, accurate documentation using Home Care Home Base (HCHB).
• Work closely with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
• Attend case conferences, staff meetings, and ongoing education sessions.
• Meet regulatory and organizational standards for documentation and care.

Qualifications:

• Valid and active RN license for the state.
• At least one year of recent home health nursing experience, preferably with field exposure.
• Proficiency in HCHB electronic medical records system.
• Demonstrated skill with OASIS assessments.
• Reliable vehicle, current drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
