Schedule:

• 13-week assignment, with potential to extend
• Monday through Friday, full-time hours
• On-call responsibility may be required
• Structured daily visit expectations

Responsibilities:

• Deliver skilled in-home nursing care in accordance with individualized patient plans
• Complete detailed assessments and OASIS documentation in a timely, accurate manner
• Perform wound care, medication management, disease monitoring, and patient/family education
• Collaborate with physicians, caregivers, and the interdisciplinary team to ensure optimal outcomes
• Communicate patient status, progress, and concerns promptly with the clinical team

Qualifications:

• Active Registered Nurse (RN) license
• At least one year of recent experience as a home health RN
• OASIS documentation experience is required
• Proficient with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Ability to adhere to a structured full-time schedule and maintain daily visit productivity

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
