We are seeking a skilled Registered Nurse (RN) with proven OASIS experience for a 13-week, full-time contract serving patients in their homes. Schedule:
- 13-week contract, with the possibility of extension
- Monday through Friday full-time schedule
- On-call duties likely required
Qualifications:
- Active RN license
- Minimum of 1 year recent experience as a home health RN
- Advanced OASIS documentation skills required
- Proficiency with HomeCare HomeBase, strongly preferred
- Reliable vehicle and valid drivers license necessary
- Ability to manage a structured daily patient visit schedule
Primary Responsibilities:
- Deliver in-home nursing visits and skilled interventions
- Complete patient assessments and OASIS documentation accurately and on time
- Collaborate with physicians and the interdisciplinary care team to implement individualized care plans
- Perform wound care, medication administration and management, and patient/family education
- Monitor disease progression and communicate updates or changes to the clinical team
