Step into a dynamic field-based nursing position focused on delivering exceptional care in patients homes. This full-time, 13-week assignment offers the opportunity to work closely with individuals and their families, utilizing clinical skills in an autonomous environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in the state of practice, in good standing
• At least one year of recent home health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• Prior experience using OASIS assessment tool
• Reliable transportation, valid drivers license, and current auto insurance

Role Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for each patient
• Provide skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient condition, identify changes promptly, and communicate updates with the multidisciplinary care team and physicians
• Educate patients and families on disease management, treatment adherence, medications, and preventive strategies
• Maintain accurate electronic records and timely documentation within Home Care Home Base (HCHB)
• Collaborate effectively with therapists, social workers, and aides to coordinate optimal patient outcomes
• Engage in case conferences, staff meetings, and required continuing education sessions
• Adhere to all regulatory and quality standards governing home health care

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
