Step into a dynamic, field-based nursing position focused on delivering high-quality home health care. This full-time, 13-week contract offers the chance to make a significant impact on patient well-being while working within a structured weekday schedule.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active, unrestricted RN license in the state of practice
• At least 1 year of recent home health nursing experience (field preferred)
• Skilled in Home Care Home Base (HCHB) electronic documentation
• Well-versed in OASIS assessments and care planning
• Reliable transportation with valid drivers license and current auto insurance

Role Responsibilities:

• Complete in-depth OASIS assessments and craft patient-centered care plans
• Provide skilled nursing services, such as wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient status, recognize any changes in condition, and collaborate with medical professionals for timely interventions
• Educate patients and families about diagnoses, medications, and continuing care needs
• Ensure accurate, timely entry of patient information in HCHB
• Work closely with other cliniciansincluding therapists and social workersto coordinate comprehensive care for each patient
• Attend case conferences, team meetings, and remain up-to-date with ongoing education
• Consistently follow all regulations and documentation standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
