Job Description

Take your nursing expertise out into the community as a Registered Nurse specializing in home health. This full-time, 13-week contract offers the opportunity to deliver compassionate care while working independently throughout the assigned territory.

Schedule & Overview:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: Travel to patient homes daily within assigned area
• Caseload: 30 points per week (typically 35 patient visits per day)

Key Responsibilities:

• Perform thorough OASIS patient assessments and develop personalized care plans
• Provide a full scope of skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, identify changes in condition, and coordinate care updates with the interdisciplinary team
• Educate patients and their families on disease management, medications, and preventive care strategies
• Accurately document all visits and procedures in Home Care Home Base (HCHB)
• Collaborate with therapists, social workers, and aides to ensure seamless care
• Attend staff meetings, case conferences, and participate in ongoing education
• Adhere to regulatory guidelines and documentation standards

Qualifications:

• Active RN license (state of practice) in good standing
• At least 1 year of recent home health nursing experience (field preferred)
• Strong proficiency in Home Care Home Base (HCHB) and OASIS documentation
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
