An excellent opportunity awaits an experienced Registered Nurse (RN) with a strong background in home health care. This contract role involves providing compassionate and skilled nursing services to patients in their residences, ensuring the highest standards of care and patient satisfaction.

Qualifications:
- Current, active RN license
- Minimum three months of home health experience utilizing OASIS documentation
- HCHB (Homecare Homebase) experience is highly desirable
- Proficiency in EMR systems and accurate patient care documentation
- Demonstrated clinical skills, including IV therapy, wound care, and medication management
- Strong communication and interpersonal abilities
- Able to work independently, demonstrating strong time management skills
- Valid drivers license and reliable transportation
Responsibilities:
- Deliver comprehensive nursing care to patients in their homes
- Conduct initial and ongoing patient assessments, including vital signs, wound checks, and medication administration
- Develop and implement tailored care plans according to patient needs
- Educate patients and families about disease management, treatment regimens, and healthy lifestyle practices
- Collaborate efficiently with physicians, therapists, and other healthcare professionals for coordinated care
- Accurately document patient care activities in EMR systems
- Closely monitor patients and promptly communicate changes in condition to supervising physicians
- Adhere to all home health care regulations at the state and federal levels
