Your next nursing opportunity starts herea dedicated Registered Nurse (RN) is needed for a full-time, 13-week home health contract, specializing in delivering compassionate care and clinical excellence to patients in their homes. Your expertise in OASIS documentation and ability to maintain a structured schedule will directly impact patient outcomes.
Schedule:
- 13-week contract, full-time
- Monday through Friday schedule
- On-call responsibilities may be required
Role Responsibilities:
- Provide skilled in-home nursing care and clinical interventions
- Complete accurate and timely OASIS assessments and documentation
- Partner with physicians and interdisciplinary teams to implement individualized care plans
- Deliver wound care, manage medications, patient education, and monitor disease progression
- Report significant changes in patient condition to the clinical team promptly
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Experience with OASIS documentation is essential
- Proficiency with HomeCare HomeBase is strongly preferred
- Demonstrated ability to manage a busy caseload and maintain documentation standards
- Reliable transportation and valid drivers license are required
