Your next nursing chapter starts with a rewarding opportunity to deliver high-impact care in patients homes. We are seeking a Registered Nurse with strong home health skills and OASIS expertise for a 13-week, full-time contract. Youll work Monday through Friday, manage a set schedule, and provide personalized care while enjoying the support of a collaborative, professional team.
Schedule:
- Full-time, Monday through Friday
- On-call rotation likely
Key Responsibilities:
- Deliver hands-on, in-home nursing care and skilled clinical interventions
- Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans
- Conduct wound care, manage medications, educate patients, and monitor for disease progression
- Clearly communicate status updates and changes in patient condition to the appropriate clinical staff
Qualifications:
- Current and active RN license
- At least one year of recent RN experience in home health
- OASIS documentation proficiency
- Familiarity with HomeCare HomeBase strongly preferred
- Organized and comfortable managing a robust daily patient visit schedule
- Valid drivers license and reliable transportation required
