Your next nursing opportunity starts here. We are seeking a compassionate and skilled Registered Nurse with expertise in home health and OASIS documentation to fill a 13-week full-time contract. This role allows you to deliver personalized care in patients homes and collaborate with a dynamic, professional care team.
Schedule:
- 13-week contract, with the possibility of extension
- Full-time hours, Monday through Friday
- On-call responsibilities are probable
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled clinical interventions
- Conduct comprehensive patient assessments and complete accurate, timely OASIS documentation
- Implement physician-ordered care plans through effective collaboration with physicians and caregivers
- Perform wound care, medication administration, patient education, and chronic disease monitoring
- Clearly communicate clinical updates and changes in patient status to the interdisciplinary team
What Were Looking For:
- Current RN license in good standing
- At least one year of recent experience as a home health RN
- Demonstrated OASIS proficiency
- Strong knowledge of HomeCare HomeBase (preferred)
- Comfortable managing a full-time, structured daily visit schedule
- Reliable transportation and a valid drivers license
