Seeking an experienced Registered Nurse skilled in Home Health to deliver compassionate, patient-centered care across a full-time, 13-week contract. This field-based opportunity focuses on making a meaningful difference by supporting patients in their homes.

Schedule and Setting:

• Monday through Friday, 8:00 AM 5:00 PM
• Expectation of 30 points per week (35 patient visits per day)
• Field-based with daily travel throughout an assigned territory (mileage reimbursement included)

Key Responsibilities:

• Perform comprehensive OASIS assessments; develop individualized care plans based on patient needs
• Deliver skilled nursing interventions, such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor and evaluate patient progression, reporting any changes promptly to physicians and the care team
• Educate patients and caregivers about disease management, medications, and preventive measures
• Complete accurate and timely documentation using Home Care Home Base (HCHB)
• Coordinate with interdisciplinary team members to provide cohesive, high-quality care
• Participate actively in case conferences, team meetings, and ongoing educational programs
• Uphold all regulatory and agency standards regarding care delivery and documentation

Qualifications:

• Active and unrestricted RN license for the state of practice
• Minimum of one year of recent Home Health nursing experience (field experience strongly preferred)
• Proven OASIS documentation proficiency
• Hands-on experience with Home Care Home Base (HCHB)
• Reliable transportation, a valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
