A full-time, 13-week contract is available for a dedicated Registered Nurse (RN) specializing in home health. Excellent clinical skills, accurate documentation, and confidence in managing a full caseload will be the keys to success in this vital role. This is an ideal position for an RN seeking rewarding work with a supportive clinical team and meaningful patient interactions.

Schedule:

  • Monday through Friday, regular business hours
  • On-call responsibilities may be required
  • Structured daily visit expectations

Key Responsibilities:

  • Provide direct in-home patient care and carry out skilled nursing interventions
  • Complete thorough patient assessments and OASIS documentation accurately and efficiently
  • Partner closely with physicians and fellow care providers to build personalized care plans
  • Perform tasks including wound care, patient education, medication management, and disease monitoring
  • Communicate critical updates and care changes promptly to the clinical team

Qualifications:

  • Active and unrestricted RN license
  • Minimum of one year of recent home health experience as an RN
  • Demonstrated proficiency completing OASIS documentation
  • HomeCare HomeBase experience strongly preferred
  • Consistent reliability in attending to scheduled daily patient visits
  • Valid drivers license with access to reliable transportation
