A dedicated Registered Nurse (RN) with home health expertise is needed for a full-time, 13-week contract opportunity in Norwalk, CT. Key Responsibilities:

  • Communicate patient status updates and condition changes to the clinical team

Qualifications / Desired Experience:

  • Active RN license
  • Minimum one year of recent home health nursing experience
  • Proficiency in OASIS documentation is required
  • Experience with HomeCare HomeBase is strongly preferred
  • Ability to manage a full-time, structured schedule with daily patient visits
  • Reliable transportation and a valid drivers license
