A healthcare facility is seeking a Registered Nurse experienced in Home Health for a full-time, 13-week contract. This field-based position focuses on delivering high-quality patient care in their homes and promotes comprehensive care planning, collaboration, and timely documentation.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapies, and chronic disease management.
• Closely monitor patient status, report changes to the care team and physicians, and adjust care plans as needed.
• Educate patients and their families on health conditions, treatment protocols, medication management, and preventive measures.
• Maintain thorough and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with interdisciplinary team membersincluding physical therapists, occupational therapists, social workers, and home health aidesfor coordinated care.
• Participate in case conferences, staff meetings, and required continuing education activities.
• Consistently follow all regulatory and best practice standards for patient care.

Qualifications:

• Current, active RN license in good standing for the state of practice.
• At least one year of recent Home Health nursing experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments.
• Reliable transportation, active drivers license, and current auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
