We are seeking a dedicated Registered Nurse (RN) with strong home health experience and proficiency in OASIS documentation for a 13-week, full-time contract. This role offers the chance to make a meaningful difference in patients lives while working alongside a professional, supportive care team.
Schedule:
- 13-week contract with potential for extension
- Full-time, Monday through Friday
- On-call responsibilities probable
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions for diverse patient needs
- Conduct comprehensive patient assessments and complete OASIS documentation with accuracy and timeliness
- Collaborate with physicians and care teams to implement and adjust individualized care plans
- Perform wound care, medication administration, disease management, and patient/caregiver education
- Communicate patient updates and changes in condition effectively to the clinical team
Qualifications:
- Active RN license required
- Minimum of one year recent experience in home health as an RN
- Hands-on OASIS experience is a must
- Familiarity with HomeCare HomeBase highly preferred
- Strong clinical judgment and organizational skills; able to manage a consistent daily visit schedule
- Reliable transportation and valid drivers license essential
