Youll play a crucial role in helping patients thrive at home through attentive, compassionate care and meticulous clinical documentation.

Schedule:

• Full-time, Monday through Friday
• On-call assignments are probable
• Daily structured patient visit expectations

Key Responsibilities:

• Deliver skilled nursing care and interventions in patients homes
• Conduct thorough assessments and complete all OASIS documentation accurately and on schedule
• Collaborate effectively with physicians and interdisciplinary care teams to implement individualized care plans
• Provide wound care, medication management, patient education, and monitor chronic disease processes
• Communicate patient status updates and any changes in condition promptly to the clinical team

Required Qualifications:

• Active RN license
• At least one year of recent home health RN experience
• Proficient with OASIS documentation
• Experience with HomeCare HomeBase strongly preferred
• Reliable transportation and a valid drivers license
• Organized and able to manage full-time, structured schedules
