Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Core Responsibilities:

• Complete OASIS assessments and craft individualized care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and communicate status changes to physicians and the care team.
• Provide thorough patient and family education on treatments, medications, and preventive strategies.
• Ensure accurate documentation in Home Care Home Base (HCHB).
• Collaborate with therapy, social work, and aide teams to deliver holistic patient care.
• Join in clinical conferences and ongoing staff development.
• Adhere to required regulatory and clinical standards throughout documentation and care delivery.

Qualifications:

• Valid, active RN license in Pennsylvania (or compact equivalent).
• At least 1 year of recent field-based Home Health RN experience.
• Proficient with HCHB (Home Care Home Base).
• Experience with OASIS required.
• Reliable transportation with valid drivers license and insurance.
