Deliver exceptional care to patients in the comfort of their homes as an experienced Registered Nurse on a full-time, 13-week contract assignment. This rewarding role is field-based, providing vital services and support throughout the community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans based on patient needs.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, promptly report any changes in condition, and collaborate with interdisciplinary team members.
• Document all care activities accurately and efficiently using the Home Care Home Base (HCHB) electronic system.
• Educate patients and their families about ongoing treatment, medication management, preventive care, and disease processes.
• Partner with physical and occupational therapists, medical social workers, and home health aides to coordinate comprehensive patient care.
• Actively participate in case conferences, staff meetings, and ongoing professional development.
• Ensure compliance with agency protocols and regulatory standards at all times.
• Travel daily within the assigned territory to complete 35 patient visits per day (mileage reimbursement provided).

Qualifications:

• Current, active RN license (state-specific and in good standing)
• At least one year of recent home health field experience
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable vehicle, valid drivers license, and current auto insurance

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
