Embark on a rewarding nursing role delivering compassionate care directly to patients in their homes through a 13-week, full-time contract. If youre an experienced Home Health RN who thrives in independent, field-based work and values meaningful patient connections, this opportunity aligns with your expertise.
Schedule & Setting:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time (13-week contract)
- Field-based: daily travel to patient homes within an assigned territory (mileage reimbursement provided)
- Caseload: 30 points per week (typically 35 patient visits per day)
What youll do:
- Conduct thorough OASIS assessments, developing personalized care plans
- Deliver skilled nursing interventionswound care, IV therapy, medication administration, and chronic disease management
- Monitor patient progress, promptly communicating changes to the healthcare team
- Provide patient and family education on disease processes, medication management, and prevention
- Collaborate with physical, occupational therapists, medical social workers, and home health aides for comprehensive, coordinated care
- Maintain up-to-date and accurate documentation using Home Care Home Base (HCHB)
- Participate in case conferences, team meetings, and continuing education
- Ensure adherence to regulatory guidelines and standards of care
What were seeking:
- Active, unrestricted RN license in the state of practice
- Minimum 1 year of recent Home Health experience (field experience strongly preferred)
- Proficient in Home Care Home Base (HCHB)
- Experience with OASIS required
- Reliable transportation, valid drivers license, and current auto insurance

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
