This full-time, 13-week assignment is structured to provide both autonomy and meaningful patient interaction, with the backing of an experienced support team.

Schedule and Location:

• Full-time contract: 13 weeks
• Monday Friday, 8:00 AM 5:00 PM
• Field-based position requiring daily travel to patient homes within an assigned territory

Key Qualifications:

• Active, unrestricted RN license in the state of practice
• At least one year of recent Home Health field experience
• Proficiency with Home Care Home Base (HCHB) documentation system
• Strong OASIS assessment skills
• Valid drivers license, reliable vehicle, and current auto insurance

Primary Responsibilities:

• Perform comprehensive OASIS admissions and recertification visits
• Develop and implement tailored care plans based on patient assessment findings
• Deliver skilled nursing interventions including wound management, IV therapy, medication administration, and chronic disease education
• Monitor and document changes in patient status, collaborating closely with the care team
• Educate patients and their families about diagnoses, therapy, and preventive practices
• Accurately complete all charting and updates in the HCHB platform
• Coordinate services with physical/occupational therapists, social workers, and aides
• Engage in team meetings and continuing education as required
• Ensure care adheres to regulatory standards
