Deliver essential care within the comfort of patients' homes by joining a team focused on patient-centered nursing. This contract role calls for a dynamic Registered Nurse with home health experience, dedicated to empowering individuals across all ages through comprehensive case management and clinical services.
Schedule/Hours:
- Must be available for weekend, holiday, and on-call rotations per department needs
- Weekend rotations occur once every 6 weeks
Qualifications:
- Minimum of 1 year of certified home care experience
- At least 1 year of case management experience
- Current, active Registered Nurse license in Massachusetts
- Reliable, independent transportation for home visits
- Flexibility and willingness to participate in weekend, holiday, and on-call schedules
Key Responsibilities:
- Deliver clinical nursing care to patients ranging from newborns to seniors in their homes
- Conduct thorough patient assessments and develop individualized care plans
- Offer education and support to patients and family members to promote wellness and understanding
- Manage the admissions process and fulfill ongoing case management duties according to established standards and policies
- Maintain timely, comprehensive electronic documentation and foster clear interdisciplinary communication to ensure holistic, coordinated care
