This contract opportunity seeks a skilled Registered Nurse with proven home health expertise to provide patient-centered care, implement effective care plans, and coordinate with an interdisciplinary team.
Requirements:
- Active RN license
- At least 1 year of certified home care nursing experience
- Minimum 1 year of case management experience
- Strong assessment, care planning, and patient education skills
- Ability to travel to patient homes as assigned
- Reliable documentation and electronic record-keeping abilities
- Commitment to delivering care aligned with professional standards and organizational policies
Schedule:
- Weekend rotation required (one weekend every 6 weeks)
Key Responsibilities:
- Assess patient status and develop individualized care plans
- Perform admission processes according to established protocols
- Manage ongoing caseload and deliver comprehensive case management
- Provide support, resources, and education to patients and their families
- Ensure accurate, timely documentation in electronic health records
- Collaborate with interdisciplinary team members for optimal care coordination
- Adhere to community health best practices, policies, and standards
