We are seeking an experienced RN with a background in home health and demonstrated OASIS expertise for a 13-week, full-time commitment. Schedule:
- Full-time: Monday through Friday
- On-call duties may be required
- Daily patient visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing care, including assessments, wound care, medication management, and disease monitoring
- Complete OASIS documentation promptly and precisely after each visit
- Collaborate seamlessly with physicians and care teams to develop and carry out individualized care plans
- Educate patients and families, supporting ongoing self-management and positive health outcomes
- Communicate patient condition changes to the clinical team as required
Qualifications:
- Active RN license in good standing
- At least one year of recent home health RN experience
- OASIS documentation experiencerequired
- Familiarity with HomeCare HomeBase (highly preferred)
- Dependable transportation and a valid drivers license
- Consistent ability to manage a structured, full-time home health schedule
