Job Description
An excellent opportunity is available for an experienced Registered Nurse with a strong background in home health care to work on a contract basis. Candidates with a passion for patient care who thrive in autonomous settings are encouraged to apply. OASIS experience is required.
Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes
- Conduct initial and ongoing patient assessments, including monitoring vital signs, managing medications, and providing wound care
- Develop and implement individualized care plans tailored to each patients needs
- Educate patients and their families on treatment plans, disease management, and preventive healthcare
- Collaborate closely with physicians, therapists, and healthcare professionals to coordinate care
- Accurately document patient care activities using electronic medical records (EMR)
- Recognize and promptly report changes in patient condition to supervising providers
- Maintain compliance with all state and federal home health care regulations
Schedule:
- Contract role; specific duration to be discussed during interview
Qualifications:
- Active RN license
- Minimum of three months of home health experience with OASIS documentation
- HCHB (HomeCare HomeBase) experience strongly preferred
- Strong clinical skills including IV therapy, wound care, and medication management
- Proficiency using electronic medical record systems
- Excellent interpersonal and communication skills
- Demonstrated ability to work independently and manage a busy schedule efficiently
- Valid drivers license with reliable transportation
