Schedule:
- Full-time, Monday through Friday
- On-call hours may be required
Essential Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Conduct thorough patient assessments, completing OASIS documentation accurately and on schedule
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Perform wound care, medication administration, health teaching, and chronic disease monitoring
- Communicate patient condition updates and care changes to the broader clinical team
Desired Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Proficiency with OASIS documentation is mandatory
- Experience with HomeCare HomeBase is highly preferred
- Strong organizational skills to manage a structured, daily visit schedule
- Reliable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
